Regenerative Weichgewebematrix
Die mucoderm® ist eine natürliche, dreidimensionale Typ I/III Kollagenmatrix, die aus der porkinen Dermis hergestellt wird. Der mehrstufige Reinigungsprozess liefert ein Endprodukt aus reinem Kollagen und Elastin, welches zudem frei von antigenen und nicht kollagenen Bestandteilen ist. Weiterhin ermöglicht die mucoderm® die Revaskularisierung und schnelle Weichgewebeintegration und stellt eine geeignete Alternative für die Entnahme patienteneigenen Gewebes dar.

Fixierung

Um die Matrix angemessen zu befestigen, sollte ein enger Kontakt zwischen der mucoderm® und dem periostalen Wundbett bestehen. Die Matrix kann dann mit Einzelknopf- oder Kreuznähten an dem Periost fixiert werden.

Vermeidung von spitzen Kanten

Nach der Rehydratation sollten die spitzen Kanten der Matrix abgeschnitten werden, um das Weichgewebe beim Verschließen des Lappens zu schützen.

Tunneltechnik

Für die Tunneltechnik wird eine verlängerte Rehydratation von etwa 10-20 min für die mucoderm® empfohlen. Die Fixierung der Matrix kann mittels Einzelknopf- oder Kreuznähten erfolgen.

Gingivarezessionen

Eine Exposition der mucoderm® Matrix sollte bei Rezessionsdeckungen stets vermieden werden. Es muss sicher gestellt werden, dass die mucoderm® durch den repositionierten Lappen vollständig bedeckt wird. Nur ein vollständiger Verschluss gewährleistet das Durchwachsen von Blutgefäßen und die Integration in das umliegende Weichgewebe. Eine frühe Exposition hingegen, kann zum Verlust der Weichgewebstransplantates führen.

Straumann® Emdogain®

Gingivarezessionen können mit mucoderm® und Straumann® Emdogain® gemeinsam behandelt werden. Die möglichen Vorteile von Straumann® Emdogain® bei der Wurzeldeckung mit mucoderm® werden u.a. in "Kollagenmatrix und Schmelzmatrixproteine" beschrieben. Dabei ist darauf zu achten, dass Straumann® Emdogain® auf die gesamte Wurzeloberfläche aufgetragen wird, bevor die mucoderm® Matrix darüber platziert wird.

Nach dem Einsetzen von mucoderm® in den Defekt dringt das Patientenblut schnell in das Kollagennetzwerk ein und lockt körpereigene Zellen zur Matrix, die die Revaskularisierung einleiten. Die Geschwindigkeit der Revaskularisierung hängt dabei von dem Gesundheitszustand des Patienten ab. Für eine Vielzahl unterschiedlicher Indikationen bietet die mucoderm® eine sichere Alternative zum autologen Bindegewebetransplantat.
